Do you have to manually turn it off, or does it automatically turn off after a certain amount of inactivity?

it will automatically turn off after a certain amount of inactivity, it is about 15 minutes.

Weird question for you guys. If I plug this into a headphone coupler, would it make my headphones Bluetooth?

I had to test this, and yes, if you plug headphones directly into it, it works. Works pretty good too.

If I use as BT receiver can I have a call using smartphone-headphone? Does the mic of smartphone-headphone work or is there any built-in mic?

As long as they connect to each other I don't see why not. I use this to stream from my phone to a surround sound and also to leave my phone in a spot charging and plug headphones into the mpow. Bam wireless headphones.
